Abstract
Adnexal masses can occur at any age. Depending on age at diagnosis and the activity of the hormonal axis, various differential diagnoses must be considered. Malignant tumors are extremely rare and account for approximately 1% of malignant pediatric tumors in girls. While epithelial tumors are the most common tumor entity in adult females, they rarely occur in prepubertal girls. Here, germ cell tumors and sex cord stromal tumors are most common. However, most adnexal findings are benign. Ovarian cysts may be diagnosed before birth; they may be simple or complex, and occasionally hormone producing. During puberty, the number of differential diagnoses increases, and in addition to neoplastic findings, ectopic pregnancies, anomalies of the Mullerian duct system, and pelvic inflammatory disease (PID) must be considered. In cases of adnexal torsion, prompt diagnosis and treatment is mandatory. Whenever possible, organ-preserving surgery should be performed with regard to future fertility. A multidisciplinary approach should be sought.
