Accuracy of conventional diagnostic methods in the diagnosis of congenital heart defects in dogs

Abstract
The aim of the study was to test the diagnostic accuracy of conventional diagnostic methods (clinical examination, electrocardiography and radiography) for congenital heart disease in dogs. A total number of 132 young dogs (less than or equal to2 years of age) with a suspected congenital cardiac anomaly were involved in a three-year period. With the aid of history, clinical examination, electrocardiography and radiography a single or multiple congenital heart defects were diagnosed or excluded. These results were compared to the results of a echocardiographic and Doppler-echocardiographic examination. In 24/132 dogs there was no congenital cardiac anomaly. The sensitivity and specitivity for the exclusion of a congenital defect using conventional methods was 8% and 100%. In 108 dogs a total number of 132 congenital defects were diagnosed by ultrasound examination. 86 (65%) of these defects were also found by conventional methods, but 46 (35%) were not. In 15 cases a wrong diagnosis was made. The most frequent defects and the sensitivity and specitivity of the conventional methods were: aortic stenosis, 53 cases (77% and 89%); pulmonic stenosis, 42 cases (43% and 100%); patent ductus arteriosus,17 cases (100% and 100%) and mitral valve dysplasia with insufficiency, nine cases (33% and 98%). The following rare congenital defects were diagnosed correctly: two of four tricuspid valve dysplasias with insufficiency, two of four atrial septum defects, two of two ventricular septum defects, one of one tetralogy of Fallot and zero of one mitral valve dysplasia with stenosis.
